If you are the authorized, designated researcher for a DNA participant (including your own DNA profile), you will be directed to complete the DNA Information Form after completing this form.

If you have already completed this form, or already registered on our site, and need to submit your DNA profile, or an additional profile for which you are the designated researcher for, please proceed directly to the DNA Information Form
